一、招標條件

本密山市人民醫院進口醫療設備采購項目（招標 (1) For foreign and overseas bidders, relevant documents registered in the place where they operate according to the laws of that country (region);
（2）對于國內投標人，若為生產企業，必須具有所投產品醫療器械生產許可證或第一類醫療器械生產備案憑證（國外制造商除外）；若為代理企業，必須具有第二類、三類醫療器械經營許可證或第二類醫療器械經營備案憑證，并提供所投產品醫療器械生產許可證或第一類醫療器械生產備案憑證（國外制造商除外）。投標產品必須具有醫療器械注冊證或第一類醫療器械備案憑證。
（2)For the domestic bidder, if it is a manufacturer, it must have the medical device production license or the record certificate of class I medical device production (excluding foreign manufacturers);If it is an agent enterprise, it must have business license of Class II or Class III medical devices or business record certificate of Class II medical devices, and provide production license of medical devices of the invested products or production record certificate of Class I medical devices (except foreign manufacturers).The bid products must have registration certificate of medical device or filing certificate of Category I medical device.
（3）擬參加本項目的投標人應具備《中華人民共和國政府采購法》第二十二條規定的資格條件：
(3) The bidder who intends to participate in the project shall meet the qualifications stipulated in Article 22 of the Government Procurement Law of the People's Republic of China:
1.具有獨立承擔民事責任的能力；
1. Ability to bear civil liability independently;
2.具有良好的商業信譽和健全的財務會計制度；
2. Having good commercial reputation and sound financial accounting system;
3.具有履行合同所必需的設備和專業技術能力；
3. Have the necessary equipment and professional and technical ability to perform the contract;
4.有依法繳納稅收和社會保障資金的良好記錄；
4. Have a good record of paying taxes and collecting social security funds according to law;
5.參加政府采購活動前三年內，在經營活動中沒有重大違法記錄；
5. Within three years before participating in government procurement activities, there were no major illegal records in business activities.
6.法律、行政法規規定的其他條件；
6. Other conditions prescribed by laws and administrative regulations;
（4）擬參加本項目的投標人須在黑龍江省 上注冊登記并備案合格；
(4) Bidders who intend to participate in this project must register and file qualified online in government procurement of Heilongjiang Province;
（5）擬參加本項目的投標人所投進口產品須具有合法來源證明文件；
(5) The imported products invested by the bidders who intend to participate in the project must have a certificate of legal origin;
（6）參加政府采購的供應商應提供參加政府采購活動近3年內在經營中無重大違法記錄的聲明或承諾；
(6) Providers participating in government procurement shall provide declarations or promises that they have no major illegal records in their operations during the past three years of participating in government procurement activities;
（7）擬參加本項目的投標人、法定代表人及授權委托人均未被司法機關列入失信被執行人名單，以中華人民共和國最高人民法院官方網站（shixin.court.gov.cn/index.html）查詢結果為準，被列入上述名單的投標人不得參加本項目；
(7) The bidders, legal representatives and authorized trustees who intend to participate in the project have not been listed by the judicial organs as the Executees of dishonesty. The results of the inquiry on the official website of the Supreme People's Court of the People's Republic of China (shixin.court.gov.cn/index.html) shall prevail. Bidders who are listed in the above-mentioned list shall not be allowed to participate in the project. ;
（8）擬參加本項目的投標人未被列入企業經營異常名錄、重大稅收違法案件當事人名單、政府采購嚴重違法失信名單，以信用中國官方網站 查詢結果為準，被列入上述名單的投標人不得參加本項目；
(8) Bidders who intend to participate in this project are not included in the list of abnormal business operations of enterprises, the list of parties involved in major tax violations, and the list of serious violations and dishonesty in government procurement. The results of the inquiry on the official website of China ( ) shall prevail. Bidders who are included in the above list shall not be allowed to participate in this project.
（9）與招標人存在利害關系可能影響采購公正性的法人、其他組織或者個人，不得參加投標；單位負責人為同一人或者存在控股、管理關系的不同單位，不得同時參加同一標包投標或者未劃分標包的同一項目投標；集團公司中母公司與子公司不得同時參加同一標包投標或者未劃分標包的同一項目投標；同一公司具有獨立法人的子公司同時參加同一標包投標或者未劃分標包的同一項目投標時最多不得超過兩家（以登記的先后順序為準）；
(9) Legal persons, other organizations or individuals with an interest in the tenderer who may affect the fairness of the procurement shall not participate in the bidding; the person in charge of the unit shall be the same person or the different units with a controlling or management relationship, and shall not participate in the same tender or the same project tender without dividing the tenders at the same time; and the group company shall not participate in the same tender or the same project tender without dividing the tenders. The parent company and subsidiary company shall not participate in the same tender or the same project tender without dividing the tender at the same time; the subsidiary company with independent legal person of the same company shall not participate in the same tender or the same project tender without dividing the tender at most two (according to the order of registration);
（10）未領取招標文件不可以參加投標。
（10）No bidding is allowed without the tender documents.
（11）本項目不接受聯合體投標。
（11）Joint bids for this project will not be accepted.

本項目不允許聯合體投標。
