normal style="text-indent:21.0pt">本項目劃分三個標段：第一標段：涉及茨芭鎮、安良鎮、白廟鄉、廣天鄉4個鄉鎮，24個安裝建設點；第二標段：涉及黃道鎮、渣園鄉、王集鄉、冢頭鎮4個鄉鎮，13個安裝建設點；第三標段：涉及堂街鎮、薛店鎮、姚莊鄉、長橋鎮4個鄉鎮，23個安裝建設點；

normal style="text-indent:21.0pt">5.5招標范圍：本標段工程量清單及施工圖設計（如有）范圍內所有內容。

normal style="text-indent:21.0pt;line-height:18.0pt">3.1持有有效的統一社會信用代碼的營業執照；

normal style="text-indent:21.0pt;line-height:18.0pt">3.2提供2021 年1月份以來任意3個月的依法繳納稅收和社會保障資金（養老保險）的相關證明材料，依法免稅的投標人應提供相應的證明文件；

normal style="text-indent:21.0pt;line-height:18.0pt">3.3 提供 2019、2020年度真實有效的財務審計報告（若投標人為新成立企業，提供自注冊年度后的財務報表）；

normal style="text-indent:21.0pt;line-height:17.0pt">3.4投標人具有電力工程施工總承包或機電工程施工總承包叁級以上（含叁級）資質，同時持有有效的企業安全生產許可證；

normal style="text-indent:21.0pt;line-height:17.0pt">3.5擬派項目經理須具有機電工程專業貳級及以上注冊建造師證，并具備主管部門頒發的安全生產考核合格證，且無在建項目（提供項目經理無在建工程承諾書，并加蓋公章由法定代表人和項目經理簽字確認）；

normal style="margin-left:21.0pt;line-height:17.0pt">3.6技術負責人須具有相關專業中級及以上技術職稱；

normal style="text-indent:21.0pt;line-height:17.0pt">3.7擬派施工員、質量員或質檢員、安全員、資料員、材料員須具有有效的崗位證書；

normal style="text-indent:21.0pt;line-height:17.0pt">3.8擬派項目經理、技術負責人、施工員、質量員或質檢員、安全員、資料員、材料員及委托代理人必須為本單位正式人員（除委托代理人外，擬派人員須一人一崗），須提供以下證明材料：①由投標人與之簽訂的勞動合同；②提供投標人為其連續繳納近3個月的養老保險證明并提供查詢頁面截圖，且可隨時查詢，采購人保留隨時核實資料真實性的權利）；

normal style="text-indent:21.0pt;line-height:17.0pt">3.9投標人須提供“中國執行信息公開網”網站的“失信被執行人”、“信用中國”網站的“重大稅收違法案件當事人名單”、“中國 ”網站的“政府采購嚴重違法失信行為信息記錄”查詢結果頁面截圖，投標人應在招標公告發布日期后對本單位信用信息進行查詢，并將查詢網頁附于投標文件中，若未按要求提供或有不良記錄，資格審查不予通過（執行財庫【2016】125號文）；

4.2投標人須將招標文件要求的資質、業績、榮譽及相關人員證明材料等資料原件掃描件（或圖片）制作到所提交的電子投標文件中。

4.3投標人對同一項目多個標段進行投標的，應分別下載所投標段的招標文件，按標段制作電子投標文件，并按招標文件要求在相應位置加蓋投標人電子印章和法人電子印章。

4.4一個標段對應生成一個文件夾（xxxx項目xx標段）,其中包含2個文件和1個文件夾。后綴名為“.file”的文件用于電子投標使用，名稱為“備份”的文件夾使用電子介質存儲，供開標現場備用。

5.加密電子投標文件的提交

5.1加密電子投標文件應在招標文件規定的投標截止時間（開標時間）之前成功提交至《全國 平臺(河南省平頂山市)》 系統（http://221.176.192.166:8080/ggzy/）。投標人應充分考慮并預留技術處理和上傳數據所需時間。
